Use this form to authorise Travelay to process a one-time payment with your card for your booking.

Important

Make sure your card details are correct, and your selfie clearly shows your face and the same card. You may cover the CVV on the back of the card with your finger or tape.

Enter your full legal name exactly as it appears on the card.

Passenger Name

If different from cardholder, enter the main passenger’s name.

Last 4 digits of card

We do not need the full card number here in writing.

Use the exact billing address registered with your card.
